Households Exiting to Permanent Housing with a Return to Homelessness within 6 Months

Measurement Period: 2023

This measure looks at the number of households who were enrolled in a program in Santa Cruz County that enters information into HMIS, moved into a permanent housing, and returned to homelessness within 6 months of securing permanent housing.

Why is this important?

Progress towards reducing and ending homelessness in Santa Cruz County requires both a reduction in the number of persons that become homeless and an increase in the rate in which persons experiencing homelessness secure permanent housing.  The County of Santa Cruz and its partner agencies are collaboratively implementing a broad set of efforts to reduce the number of persons experiencing homelessness countywide.  This indicator tracks the number of households that return to homelessness within six months of exiting to permanent housing and is a measure of the efficacy of countywide efforts to support households experiencing homelessness in retaining permanent housing.
